| Description: | Reporting to the EHR Program Manager, the Medical Abstractor collects patient information from various sources to analyze for the purposes of research activities. The Medical Abstractor abstracts, submits, validates, tracks and analyzes patient information from a range of sources, including medical records and interviews with staff and/or patients for the purpose of research activities. |
| Duties: | A. Identifies and compiles patient data from a variety of relevant sources.
B. Abstracts medical information and codes into database in accordance with established policies and procedures.
C. Reviews data for completeness and accuracy; investigates the location of missing or not readily accessible information.
D. Tracks patients and collects follow-up data as appropriate.
E. Responds to internal and external inquiries.
F. Maintains knowledge of medical terminology and documentation trends.
G. Perform other duties as assigned.

| Qualifications: | Bachelor�s degree in healthcare or related field or graduate of an approved school of medical record technology or successful completion of independent study course conducted by American Health Information Management Association (AHIMA) or high school diploma/GED and three (3) years of coding or abstracting experience. Ability to read and interpret patient charts and medical terminology. Good verbal and written communication skills, along with computer skills. |
